Our patients afflicted with diabetes types I or II are often well acquainted with giving themselves multiple subcutaneous injections of insulin several times each day. Tablets are also used in diabetics who still produce insulin, but not enough to keep blood glucose at optimal levels. While these medications are nothing short of life-sustaining miracles, the constant, bothersome and often painful regimen of multiple pills and/or injections often becomes more of a punishment than treatment, and leads to poor compliance with treatment plans and dosing strategies. A new, viable alternative to pre- meal injections and/or tablets is inhaled low-dose insulin. Exubra[R] is an inhaled insulin that has proven to be effective in long term studies (Rosenstock, et al, 2005; Pfizer, 2006), and is a reasonable alternative, in some cases, to injected or by-mouth forms of insulin for select patients with diabetes I or II. Exubra[R] does NOT replace slow acting insulin given by injection once or twice daily (Rosenstock et al, 2005, 549). It is developed to be taken 10 minutes before meals (pre-prandial) to help control the surge in blood glucose that comes with digestion of carbohydrates. The combination of slow acting (ultralente) insulin and Exubra[R] proved effective in several controlled studies (Collazo-Clavell, 2007, 1; Pfizer, 2006,3).
